Cagrilintide

Long-acting amylin analogue that works through a satiety pathway separate from GLP-1 - which is why it combines with incretins instead of duplicating them.

Investigational. Studied mainly as CagriSema, a fixed combination with semaglutide, in Novo Nordisk's REDEFINE programme. Not approved.

What it is

Amylin is a hormone co-secreted with insulin by pancreatic beta cells. It signals satiety and slows gastric emptying through pathways in the hindbrain that are distinct from GLP-1 signalling. Cagrilintide is a long-acting synthetic amylin analogue engineered for weekly dosing.

Why it matters

The reason cagrilintide gets attention is combination, not monotherapy. Because it works on a different satiety pathway, adding it to semaglutide is genuinely additive rather than redundant - unlike stacking two incretins, which mostly stacks side effects.

CagriSema (cagrilintide + semaglutide) trials have reported weight reductions in the low-to-mid twenties percent at 68 weeks, putting the combination in retatrutide territory using two well-understood mechanisms instead of one novel triple agonist.

Practical notes

  • Alone, cagrilintide produces meaningful but unremarkable weight loss. It is a combination drug in practice.
  • Nausea is not additive in a friendly way. Running cagrilintide alongside a GLP-1 means two satiety mechanisms and two nausea sources; titration of the pair has to be slower than titration of either alone.
  • Grey-market cagrilintide is common precisely because the approved combination does not exist yet, and it carries every sourcing problem described in safety and sourcing.
