The most widely used healing peptide in the world, supported by a large rodent literature and zero completed randomised human trials.
Preclinical only
Not approved anywhere. In 2023 the FDA placed BPC-157 in Category 2 of its bulk drug substances review - substances with significant safety concerns - barring compounding pharmacies from producing it. WADA-prohibited in sport.
What it is
BPC-157 is a 15-amino acid sequence derived from a protein found in human gastric juice. It is the default answer in every peptide community to "I have a nagging injury," and it is the clearest example on this site of the gap between how much a compound is used and how much is actually known about it.
The honest evidence position
There is a large preclinical literature - well over a hundred rodent papers reporting accelerated healing of tendon, ligament, muscle, bone, gut mucosa and nerve, plus protective effects across a strange breadth of injury models.
There is not a single completed randomised controlled trial in humans. A trial was registered under the name Bepecin in 2015; it produced no approved use. Every human effect claim you will encounter is either extrapolation from rats or self-report.
This is not a claim that BPC-157 does nothing. It is a claim that nobody knows, including the people selling it - and the breadth of the rodent claims is itself a yellow flag, because compounds that fix everything in mice have a poor record of fixing anything in people.
Mechanism
BPC-157 does not act on one identified receptor. The best-supported mechanism is promotion of angiogenesis - new blood vessel formation at injury sites - through the Egr-1/NAB2 pathway, with additional reported effects on the nitric oxide system, growth factor receptor expression (notably VEGFR2), and fibroblast migration.
The angiogenesis mechanism is also the source of the main theoretical safety concern: growing new blood supply is exactly what you do not want happening near an undiagnosed tumour. This has not been demonstrated in humans. It has also not been ruled out, because nobody has looked.
Regulatory position
The FDA's 2023 decision matters more than it is usually given credit for. Placement in Category 2 means the agency identified significant safety concerns - specifically the potential to trigger an immune response, the likelihood of impurities in supplied material, and incomplete safety information. Compounding pharmacies may no longer legally produce it for human use.
Products sold as oral BPC-157 "supplements" are marketed under a different regulatory theory; the FDA has stated BPC-157 does not qualify as a dietary ingredient.
Practical notes
- Local versus systemic injection is contested. Injecting near the injury site is common practice on the theory of local concentration; the rodent data show systemic effects from remote administration, so the theory may not be necessary. No human comparison exists.
- Oral versus injectable. Oral is used mainly for gut-related complaints, on the logic that a gastric-derived peptide acting on gut mucosa does not need systemic exposure. For a tendon, oral bioavailability of a 15-mer peptide is the obvious objection.
- Blocks, not continuous use. Most reported protocols run 4-8 weeks. There is no evidence basis for that duration either - it is convention.
- Tested athletes must not touch it. BPC-157 is explicitly prohibited by WADA and USADA, and it is detectable.
The stack
BPC-157 and TB-500 are the canonical healing pair, on the reasoning that they act through different mechanisms - angiogenesis versus actin regulation and cell migration. Both are preclinical-only, so the stack is two unknowns rather than one.
Side effects and warning signs
Commonly reported
- Injection-site irritation
- Occasional reports of lightheadedness or fatigue after dosing
- No systematic human safety dataset exists - the absence of reported effects reflects absence of study, not evidence of safety
Stop and get medical help
- Any new or growing lump - angiogenesis-promoting compounds and existing malignancy is an unstudied and theoretically adverse combination
- Signs of injection-site infection: expanding redness, heat, or fever
- The FDA's stated concern was immunogenicity and impurities; an unexpected systemic or allergic reaction warrants stopping
